Health Care Service is available in the health office. Please notify the school nurse if your child has any health concerns that could result in the need for emergency care services or that would impact their emergency care. To better meet the needs of your child, contact your child's school nurse if your child has any special health care needs or requires medication or health treatment during school.
Medication form:
This form is for any student that requires medication during the school day. This form must be filled out by your doctor and medication must be dropped off by a parent/guardian to the school nurse with this form.
Students with diagnoses such as asthma and life-threatening allergies are good examples when self-administration may be appropriate and necessary. In order to accommodate students who carry and self-administer emergency medications, the following should be in place:
- An order from a licensed prescriber for the medication, including a statement that it is necessary for the student to carry the medication and that the student is capable and competent of self-administration and written parental/guardian consent.
Emergency care plan: This form is for any student requiring an emergency care plan, such as a plan for epilepsy, diabetes, or life threatening allergies. This form is to be filled out by the parent/guardian and signed by the doctor. Please include a student photo if you are able to.
